Chronic Disease Management

Our highly trained Practice Nurses and your GP will work with you to help you manage your chronic illness and achieve your health goals. Allied Health practitioners such as physiotherapists, podiatrists and exercise physiologists can be an essential part of your chronic disease management team and we can make recommendations for practitioners both within Adelaide Health Care or close to your home.

If you have a chronic illness you may be eligible for a General Practice Management Plan and Team Care Arrangements. Discuss this with your usual GP or one of the Practice Nurses.

Children and teenagers can have chronic illnesses as well and may also be eligible for a General Practice Management Plan and Team Care Arrangement.
